Services

PK Independent Living provides NDIS-approved support services that are tailored to meet the unique needs of each individual client. The specific services provided, including the scope and duration, are detailed in the Service Agreement that is established between PK Independent Living and the client. This agreement outlines the obligations of both parties and serves as the foundation for our service delivery.

Eligibility

Our services are available to individuals who meet the eligibility criteria set by the National Disability Insurance Scheme (NDIS). To access our services, clients must provide accurate and up-to-date information regarding their NDIS eligibility and plan details. This information is crucial for ensuring that the appropriate services are provided and that NDIS funding is correctly applied.

Service Agreement

Before commencing services, a Service Agreement will be established between PK Independent Living and the client. This agreement outlines the specific services to be provided, the fees associated with those services, and the responsibilities of both parties. The Service Agreement must be signed by the client (or their authorized representative) and PK Independent Living before any services can be delivered.

Payment and Fees

The fees for our services are outlined in the Service Agreement and are typically covered by the client's NDIS plan. In some cases, clients may be required to pay additional fees for services not covered by the NDIS. Payments are due as per the terms specified in the Service Agreement. Clients are responsible for ensuring that payments are made in a timely manner to avoid any disruption in services.

Cancellations and Refunds

Clients must provide at least 24 hours' notice if they need to cancel a scheduled service. Failure to provide adequate notice may result in a cancellation fee, as outlined in the Service Agreement.

Privacy and Confidentiality

PK Independent Living is committed to protecting your privacy and maintaining the confidentiality of your personal information. We collect, use, and disclose personal information in accordance with our Privacy Policy, which complies with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. We take all reasonable measures to ensure that your personal information is securely stored and accessed only by authorised personnel.

Changes to Services

PK Independent Living reserves the right to modify or discontinue any service, provided that reasonable notice is given to the client. Any changes to services will be discussed with the client and documented in an updated Service Agreement, if necessary.

Client Responsibilities

Clients are expected to provide accurate and complete information regarding their needs and NDIS plan. They must also communicate openly with PK Independent Living about any changes in their circumstances that may affect service delivery. Clients are responsible for adhering to the terms of the Service Agreement and for promptly notifying us of any concerns or issues.​

Termination

Either party may terminate the Service Agreement with 30 days' written notice. PK Independent Living may also terminate the agreement immediately if the client breaches any of the terms and conditions outlined in this document or the Service Agreement.

Governing Law

These terms and conditions are governed by the laws of WA, Australia. Any disputes arising under these terms will be subject to the jurisdiction of the courts in WA.
